IMPORTANT INFORMATION: CONSENT TO TREATMENT
As with the transmission of any communicable disease like a cold or the flu, people may be exposed to the Coronavirus at any time or any place. As we always have, we will continue to follow state and federal regulations and universal protection precautions and disinfection protocols.
Despite our careful attention to sterilization, disinfection, and use of PPE, there is still a rare chance of exposure at our office, just as there could be at the grocery store or anywhere you may go. Social distancing nationwide has reduced the transmission of the Coronavirus. Although we have taken measures to provide social distancing, due to the nature of our services and the procedures we provide, it is not always possible to maintain social distancing between the patient, doctor, staff and sometimes other patients, parents or guardians.
Although exposure is unlikely, we are unable to safely have everyone sign a specific consent form for every patient prior to their appointment. Therefore, upon entering our office you accept this risk and consent to treatment.
“AT-RISK” PATIENTS and FAMILY MEMBERS
We have implemented strict precautionary measures prior to entering our office. We are also implementing special hours for at-risk patients and family members.
The CDC’s definition of at-risk includes any of the following–you or anyone that lives with you if someone in your household is over 65, has serious underlying health conditions including high blood pressure, diabetes, chronic lung disease, asthma, obesity, or those whose immune system is compromised.
If this pertains to you or anyone in your household, contact our office IMMEDIATELY to discuss your appointment. Delaying contacting us may create delays in finding appointment times that suit your needs.
SCREENING AND WAITING ROOM POLICIES
Entering our office will not be permitted if any of the following criteria is discovered while checking in:
- Temperature of 99.6 or higher
- Symptoms of illness, cough, fever, loss of taste or smell, sore throat, body aches, chronic headache within the last 14 days.
- Persons that are residing in the same household with anyone with above symptoms within the last 14 days.
Parents/Guardians/Providers of Transportation will not be permitted to enter the office unless the patient is under the age of 10, an initial new patient consultation is scheduled, or paperwork needs to be completed. In these instances, only one person can accompany a patient. All persons entering the office are subject to screening as above prior to entry.
If you need orthodontic supplies or need to make a payment, please call the office and we can get whatever you need ready to pick up. You can make payments over the phone.
